contextflow and the EIC Accelerator: Lung AI Sold to Australia
contextflow, founded in Vienna in 2016 as a spin-off of the Medical University of Vienna and the EU research project Khresmoi, develops AI software that helps radiologists detect and track lung cancer, interstitial lung disease and COPD on chest CT scans. It was selected in the EIC Accelerator's March 2023 cut-off for "Integrated AI-based Clinical Decision Support for Radiologists" with blended finance, and its website says it received "both grant and investment awards from the European Innovation Council". No project for this 2023 award appears on CORDIS, the EU's register of funded projects, so its amounts and results are not public.
On 1 June 2026 4DMedical, a lung imaging company listed on the Australian Securities Exchange, announced a binding agreement to buy contextflow, and contextflow says the exit was completed in August 2026. 4DMedical paid about A$18.56 million (€11.4 million) in cash upfront plus 56,235 shares and an earnout of up to 2.59 million share options, and described the terms as significantly discounted; it also retains about €19 million of accumulated tax losses, according to Trending Topics.

Where contextflow Stands Now
contextflow is now part of 4DMedical, which said the deal gives it an immediate European commercial and clinical presence with an active team, customer base and CE-certified product portfolio. Trending Topics described the sale as "more pragmatic than triumphant", given the discounted terms and the large tax losses, despite contextflow's reimbursement breakthrough in Germany. In November 2024 contextflow announced it was set to secure what it called Europe's first radiology AI reimbursement contract, with IKK Südwest, according to its announcement.
The record
From Vienna spin-out to an Australian acquisition

"As part of the acquisition, 4DMedical retains a cumulative tax loss of €19.0 million (approx. A$30.8 million), which may potentially be used to offset future taxable income from the Austrian business and represents a significant additional source of value."

contextflow's software analyses chest CT scans to find and measure lung nodules, assess malignancy and flag incidental findings such as pulmonary embolism, and can retrieve visually similar reference cases. Its products are CE-marked under the EU's Medical Device Regulation and in routine clinical use, according to the company and 4DMedical.
The price
What 4DMedical paid and what it gained
The upfront cash was about €11.4M. 4DMedical also takes over about €19M of tax losses, a measure of what contextflow lost before the sale. The earnout depends on milestones and 4DMedical's share price.

What the EIC Award Promised, and What the Record Shows
The 2023 selection was contextflow's second EIC Accelerator award. In the March 2020 cut-off of the Horizon 2020 EIC Accelerator pilot, it won a grant, without equity, for a project named Contextflow, a "3D image-based search engine and triaging software for lung CTs to improve radiology workflows", according to the European Commission's list of selected companies. The project ran from September 2020 to August 2022 with an EU contribution of €1,205,872.50 towards a total cost of €1,722,675; its final report says it developed detection of lung diseases, including COVID-19, from the patterns the software finds and tracking of those patterns in a patient over time, with the results built into ADVANCE Chest CT, according to CORDIS. contextflow's product page describes ADVANCE Chest CT as CE-marked, with nodule tracking over time and analysis of 19 image patterns.
The award was to scale integrated AI decision support for radiologists. Without a CORDIS project for the 2023 award, there is no public record of what the EIC paid, what milestones were set or whether the EIC Fund invested. The company did achieve a German reimbursement contract and partnerships with hospitals and imaging platforms, but it was sold three years later for a price the buyer called discounted.

What Cannot Be Checked
Questions for the EIC
How much the EIC paid as grant and equity, why no project record exists, and what the EIC Fund recovered in the sale to 4DMedical are not public.
Why Europe Should Care
contextflow was a European leader in radiology AI with EU roots going back to the Khresmoi research project. Its technology and team now belong to an Australian listed company, which bought it to enter the European market. EIC support helped build an asset that a non-European buyer acquired on terms it called discounted.
What the award promised
Scaling AI-based clinical decision support for radiologists reading chest CT scans.
What the record shows
A certified product with a German reimbursement deal, then a sale to an Australian company on terms the buyer called discounted.
The Verdict
A European asset, sold abroad
contextflow built a working product with EU support at several stages. It ended as an acquisition by 4DMedical on terms the buyer called discounted, and the EIC's return is unknown.
