SentryX and the EIC Accelerator: Opioid-Free Spine Surgery Relief

30 September 2026911 words5 min

SentryX, a Dutch company whose CORDIS address is in Austerlitz near Utrecht, has developed BR-003, a ring-shaped, biodegradable gelatin hydrogel that surgeons slide onto pedicle screws during spine fixation surgery, where it releases the local anaesthetic bupivacaine for more than 72 hours. The aim is to replace the high-dose opioids that patients receive after one of the most painful operations. SentryX was selected in the EIC Accelerator's January 2023 cut-off and announced €15.5 million in EIC funding; its final report describes this as "a €2.5 million grant and €13 million equity". The grant project, OPERAS, cost €5.69 million and ran from April 2023 to March 2026, according to CORDIS.

The project is closed. Its final report states that a Phase Ib trial with 11 patients in the Netherlands and Switzerland met its primary safety endpoint in every patient, with mean peak blood bupivacaine of 149 ng/mL against a 2,000 ng/mL threshold, and that no product-related serious adverse events occurred up to a year after surgery, according to the CORDIS reporting page. The safety results were published in a peer-reviewed journal, EMA scientific advice on a single adaptive Phase II/III pivotal trial was obtained in February 2026, and "all 14 deliverables and all 3 milestones of the project were achieved".

Where SentryX Stands Now

SentryX is preparing its pivotal trial and in February 2026 reported a key step for its scaled-up production line, according to its news page. It formed a supervisory board in May 2025, added four spine surgeons to an advisory board in November 2025, and won the ATiO Emerging Company Contest in June 2026. The company states that BR-003 "has not been approved by the FDA, EMA, or any other regulatory authority". No start date or financing for the pivotal trial has been announced.

The record

From sheep studies to a pivotal trial plan

SentryX completed its first-in-human trial, published the safety results and obtained EMA scientific advice during its EIC project. The pivotal trial has not yet started.

The Technology: A Ring of Slow-Release Anaesthetic

The SentryX ring keeps releasing anaesthetic for more than 72 hours, nine times longer than conventional injections, the company says, and then dissolves. It is made from an ultra-pure modified gelatin developed in-house and fits commercial pedicle screws without changing the operation. In the trial, bupivacaine stayed measurable in blood for two weeks.

The safety endpoint

Peak blood bupivacaine against the safety threshold

The primary safety endpoint was met in all 11 patients: the mean peak blood level was 13 times below the accepted threshold.

What the EIC Award Promised, and What the Record Shows

The project had three goals: a Phase Ib first-in-human safety trial, a market access strategy, and regulatory preparation. All three were completed, and the safety data was published. The efficacy findings, 42% less opioid use and 49% lower back pain than 41 standard-care patients, are exploratory: they come from a small trial compared with an observational group, not a randomised control arm.

The Money

According to the final report the EIC committed €13 million of equity alongside the grant. How much of that has been invested, and what other investors hold, is not public, and SentryX's accounts could not be checked from free sources. The company has not disclosed how it will fund the Phase II/III trial.

The early signal

Exploratory outcomes against a standard-care cohort

Compared with 41 observational patients on standard care, the 11 trial patients used 42% less opioid and reported 49% lower back pain. These are exploratory results from a small, non-randomised trial.

The EIC award

Blended finance from the EIC

The EIC award combined a €2.5M grant with €13M of equity, according to the final report.

What Cannot Be Checked

Questions for the EIC

How much of the €13 million equity has been paid in, what stake the EIC Fund holds, when the pivotal trial will start and who will fund it are not public.

Why Europe Should Care

The final report cites an 18% risk of opioid dependence after spine surgery and about 1.2 million spine fixation procedures a year in Europe and the US. A safe, non-opioid way to control that pain would matter to patients and health systems. SentryX has shown the approach is safe in a small trial; whether it works still has to be proved.

What the award promised

A first-in-human trial, a market strategy and regulatory readiness for late-stage development.

What the record shows

All three achieved, safety data published, EMA advice obtained, and a pivotal trial still to be run.

The Verdict

Milestones reported met

SentryX says it delivered every milestone of its EIC project. The costlier test, a pivotal trial, lies ahead.
